1. Try removing everything related to the interrupt handling (pinMode() , attachInterrupt() , pin*Fired() ...) while keeping counter* volatile. Does the problem persist? 2. Could you share the compiled .elf file corresponding to the minimal program that shows the problem? If so, please update the source code in the question to match this minimal program.

add some printing to setup(). something like Serial.println("START"); to see if the MCU doesn't reset for some reason.
